Application to Use Animals in Research, Teaching or Testing
The forms included in this packet are necessary for submission of a new or renewal (three-year) protocol to the IACUC.

All forms must be typed and completed using the form fields provided. In order to view the form fields, you may have to have the "show/hide" selection on. The IACUC will not review applications if the narrative text is not inserted into the form field (except in instances where attachments are requested), nor will handwritten applications be accepted.

In order to assist investigators in avoiding a lapse in approval, the IACUC Office sends reminder notices for protocols requiring a three-year renewal approximately 90 days before the end of the approval period. A "New Application Packet" must be reviewed and approved by the IACUC before the expiration of the second annual renewal.

Application to Use Animals in Euthanasia Procedures Only
The forms included in this packet may be used for submission of a new or renewal (three-year) protocol to the IACUC for projects involving euthanasia of an animal prior to removal of tissue or other manipulations. If tissue collection will be done while the animal is under anesthesia, or the euthanasia procedure is part of a larger animal study, the Application to Use Animals in Research, Teaching or Testing available at the top of this window must be submitted instead of the Application to Use Animals in Euthanasia Procedures.

Application to Establish and/or Maintain a Breeding Colony
The forms included in this packet are required for submission of a new or renewal (three-year) protocol to the IACUC in order to breed animals for subsequent transfer to an approved protocol without having performed any manipulations on the animals prior to transfer.

PLEASE NOTE: If you are planning to breed and subsequently use the animals on an individual protocol, you should describe the breeding as part of that protocol, using the Application to Use Animals in Research, Teaching or Testing, available at the top of this window, which has a section for breeding. You should not complete the Application to Establish and/or Maintain a Breeding Colony.

Application for Modifications to an Approved Protocol
The forms included in this packet are required for requesting changes to an already approved protocol. Changes include, but are not limited to: changes in personnel, laboratory location, increase in numbers of animals, change in anesthetic, change or new procedure.

IMPORTANT REMINDER:Significant Changes to approved protocols must be reviewed and approved by the IACUC prior to being implemented. See the IACUC Policy for Significant Changes to Approved Research Protocols for additional information.

Application for Annual Continuation of an Approved Protocol
Federal requirements mandate that protocols be reviewed by the IACUC and granted continuation no less than annually at the end of years 1 and 2., the Application for Annual Continuation of an Approved Protocol must be completed and submitted to the IACUC for consideration.

In order to apply for continuation of an approved protocol, the forms included in this packet must be completed and submitted to the IACUC for consideration.

IMPORTANT REMINDER: At the end of year three, you must provide a new description of proposed use of live vertebrate animals if you wish to continue your research project. To do this, you must download and complete the appropriate Application packet.

To assist investigators in avoiding a lapse in approval, the IACUC Office sends an annual reminder approximately 60 days before the end of the approval period.

Request for Research Proposal/IACUC Protocol Review
(formerly: Adding a Title/Funding Source to an Approved Protocol)

After receiving a fundable priority score, PIs should send a Request for Research Proposal/IACUC Protocol Review form to the SRA at the IACUC Office, 300 George Street, 6th Floor.

a.

If the research proposal has an associated IACUC approved protocol: Submit the Request for Research Proposal/IACUC Protocol Review form to the SRA no less than 30 days from the anticipated award date to allow time for possible required modifications to the protocol.

b. If the research proposal does not have an associated approved IACUC protocol: Submit a new IACUC protocol along with the Request for Research Proposal/IACUC Protocol Review form to the SRA no less than 60 days from the anticipated award date to allow time for the processing of a new IACUC protocol.

Application for Permission to Have Custom Antibodies Manufactured
This application covers all requests for manufacturing custom antibodies either at contract laboratories off site or here at Yale University. Please refer to the Contract Laboratory Approved List on the IACUC Website.

If the contract laboratory is noted on the approved list only the Application for Permission to Have Custom Antibodies Manufactured is required.

If the contract laboratory is not on the approved list, a Contract Laboratory Animal Use Agreement, available at the top of this window, signed by the Contract Laboratory, must be attached to this application. This application will not be approved without a signed Contract laboratory Animal Use Agreement on file.

Custom Antibody Contract Laboratory Agreement
This form is required only if the contract laboratory is not listed on the Contract Laboratory Approved List. The contract laboratory responsible for manufacturing custom antibodies must complete it.

IMPORTANT REMINDER: Contract laboratories must have an approved Assurance of Compliance with Public Health Service Policy on file with the Office of Laboratory Animal Welfare (OLAW). Contract laboratories that do not have an Assurance on file with OLAW will not be approved.

IACUC Prosum Form**
When submitting a research proposal to any funding agency PIs will submit an electronic or photo copy of the entire research proposal along with the Prosum form to the IACUC Sponsored Research Analyst (SRA) to archive for subsequent review.

The Prosum can be submitted electronically along with the proposal at IACUC@Yale.edu or as a hardcopy along with the proposal to IACUC Office, 600 George St., 6th Floor.

**It is important to note that emailing or sending a photocopy of the proposal and SRA ProSum is not the trigger for review to occur. The proposal, along with the Prosum form is archived until a Request for Research Proposal/IACUC Protocol Review form is received by the SRA.
