| Application to Use Animals in Euthanasia Procedures Only Complete this application if your project involves euthanasia of an animal prior to removal of tissue, or other manipulations. |
| Application to Establish and/or Maintain a Breeding Colony Complete this application for any breeding colonies to be established/maintained that will not be part of a specific protocol). |
| Application for Modifications to an Approved Protocol Complete this application when making changes to an already approved protocol. Changes include, but are not limited to: changes in personnel, laboratory location, increase in numbers of animals, change in anesthetic, change or new procedure. For additional requirements, see the IACUC Policy for Significant Changes to Approved Research Protocols. |

Application for Annual Continuation of an Approved Protocol Complete this application to continue IACUC approval of an already approved protocol. Federal requirements mandate that protocols be reviewed and granted continuation no less than annually in years 1 and 2. |

Request
for Research Proposal/IACUC Protocol Review (formerly
Request for Adding a Title/Funding source to an Approved Protocol) Complete this form if you are adding a research proposal (grant), funding source or fellowship application to an already approved protocol. The entire proposal should be sent to the IACUC at the time of submission to the funding agency along with an SRAProsum . Upon receipt of a fundable priority score send the Request for Research Proposal/IACUC Protocol Review to IACUC@Yale.edu or to Attention: SRA, IACUC Office, 300 George St. 6th floor. If modifications to the approved protocol are required, you will be contacted within 10 days by the sponsored research analyst. |

Application
for Permission to Have Custom Antibodies Manufactured Complete this form if you wish to apply for approval to have custom antibodies manufactured from a contract laboratory off site or here at Yale University. Be sure to check the Contract Laboratory Approved list to be sure the laboratory you wish to use is noted. If the contract laboratory is not on the approved list, you must also complete a Contract Laboratory Animal Use Agreement have it signed by the Contract Laboratory and attach it to this application. This application will not be approved without a signed Contract laboratory Animal Use Agreement on file. |

SRA Prosum Complete this form when sending a research proposal to the IACUC. All proposals should be sent to the IACUC simultaneously with submission to the funding agency. Receipt of the Prosum along with the proposal will be acknowledged via email. It is important to note that emailing or sending a photocopy of the proposal and SRA ProSum is not the trigger for review to occur. The proposal, along with the Prosum form is archived until a Request for Research Proposal/IACUC Protocol Review form is received by the SRA. |
